Submit one letter of reference from a teacher and include ONE sample of up to one page double-spaced of prose, poetry, or spoken word.

• Fill out the application form below. Be sure to include your FULL name, mailing address, email address, phone number and age (you must be between 12-18 years of age), and an e-mail address.
  
• This award is given at YouthWrite's discretion based on the quality of writing and financial need.
  
• The successful applicant(s) will be notified via e-mail.
